About the Role Promoting independence, choice and control is at the heart of this post. This role in Adult Social Care that with a newly created All Age Learning Disabilities Team. This is an important and exciting opportunity to work with people in a person-centred way, proactively seeking to empower them and maximising their potential for independence. The post will require you to travel and be based across different teams in Surrey, therefore a full driving licence and the use of a car is essential. You will be part of a team and will receive regular support and supervision from a qualified practitioner. Your role will be to support people to exercise choice and control in planning how they will use their personal budget, by providing them with information and / or helping them to arrange services so that they manage their personal budgets effectively. You will work in partnership with individuals, their carers, professional staff in other agencies, and service providers in order to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control. You will manage a workload that will include direct casework as well as networking with community and professional resources in a particular locality. An important part of the role will be developing effective working relationships with external brokers and a variety of potential support networks, or identifying the need for and developing new ones, in order to provide increased accessibility to resources and facilities that promote choice and social inclusion. You will also be required to develop intelligence on resources around the local area and supporting colleagues to formulate complex support plans. Successful applicants will have very good interpersonal skills, be creative problem-solvers, have substantial experience of working directly with people with social care needs and of visiting them in their own homes or their care setting as part of paid or voluntary work. You may have experience of community development work and/or arranging / procuring services for people. You will be confident and competent in working with people with a variety of physical and cognitive impairments. You will need to have competent IT skills and be able and willing to learn to use a variety of software systems.
